New resource to support practices take action on improving access

The RAFT locality collaboratives are a good way to learn from others about the variety of changes we can make, to improve access and manage workload. What comes next is just as important though – turning those ideas into plans and taking practical action to implement change.

To support practices with this, we have produced a RAFT planning toolkit. This online resource includes ideas, tips and interactive downloadable tools to help you design your new patient pathway and make actionable plans.
